Participant Consent

The success of a Quiet Waters’ Event depends heavily on the carefully nurtured Christian community that allows individuals living with disability to feel blessed and supported. For this reason, Quiet Waters reserves the right to accept families, volunteers, and leaders and/or assign accommodations based on our ability to meet the needs of all participants in a safe Christian environment. Thank you for your understanding in helping us preserve the community of Quiet Waters’ events.

Media Consent

As part of our application process, and by reading this and checking the box on the application form, you hereby agree and consent that Quiet Waters may take images and/or audio recordings of me and my voice, and the image and/or voice of any child(ren) for whom you are the parent or guardian, and they may be used without restriction by Quiet Waters  for any purpose, including public broadcast. 

These conditions apply to any submitted media, such as video footage, audio recordings, images and still photographs provided by me as an on-camera participant for Quiet Water’s use. It is also understood that Quiet Waters retains exclusive, unconditional ownership of and broadcast rights to any images or sound recordings obtained by Quiet Waters. 

You hereby release and discharge Quiet Waters and its licensees from any claims of damage or cause of action resulting from usage of these images and/or audio recordings. You agree that the consent provided herein is irrevocable. 

You will be given the opportunity to select which members in your group can have their image/voice recorded.
